Objective: To explore the difficulties faced by mental health professionals in conducting cognitive behavioral couple therapy with Pakistani couples showing marital dissatisfaction.&#13;
Study Design: Transcendental Phenomenological study.&#13;
Place and Duration of the Study: Study conducted during the course of one year period in the Armed forces institute of mental health, Rawalpindi Pakistan.&#13;
Method: In depth semi structured interviews were conducted with 3 psychologists and 2 psychiatrists working in Armed Forces institute of Mental Health, Pakistan.&#13;
Results: All mental health professionals conducting CBCT with Pakistani couples reported need for cultural adaptation of CBCT manuals for usage in Pakistani society. Although they were not able to elicit on these details, however four major underlying themes were revealed through thematic content analysis: obstacles in conducting CBCT, issues related to treatment module (CBCT), participation of the family members and behavioral changes observed in therapy.&#13;
Conclusions: For CBCT to be acceptable in Pakistan, numerous cultural adaptations need to be taken into consideration relating to patient’s knowledge and belief about therapy itself and factors related to services provided by mental health professionals. In depth interviews with mental health professionals can provide us with clear guidelines of development of CBCT in accordance to the needs of Pakistani couples.&#13;

Objective: The purpose of this study is to highlight the presently used medical educational principles&#13;
highlighted in the Quran, analyze them and correlate them to derive lessons facilitating their implementation.&#13;
Study Design: Mixed method study.&#13;
Place and Duration of Study: Department of Medical Education, CMH Medical College, Lahore; January to&#13;
December, 2013.&#13;
Material and Methods: Verses of Quran from different Tafaseer (Explanation of Quran by renowned Muslim&#13;
Scholars) related to education were identified. Help from dictionaries and encyclopedias available on internet&#13;
were sought for true meanings and connotations. The ideas derived from the medical educational research&#13;
were applied to find educational principles and compared to existing medical educational philosophies.&#13;
Learner response system was used to gather educationalists opinions.&#13;
Results: Principles of medical education which have been learned over last few decades and are now globally&#13;
accepted and practiced were already there in Quran for last 1400 years. The common principles discovered&#13;
and discussed were: Simplicity, Learning in steps, Paced learning, Identification of important, Critical&#13;
thinking, Inductive reasoning, Use of examples, and Repetition. Most of these are core principles of medical&#13;
education.&#13;
Conclusion: Adult learning principles are already present in the Quran. In the propagation of these modern&#13;
medical educational principles, references to Quran may help to understand their deeper perspectives. It may&#13;
expedite the wider acceptance by educationalists in the Muslim countries.&#13;

OBJECTIVE: To determine frequency of metabolic syndrome in patients with Schizophrenia, in&#13;
a tertiary care armed forces setup.&#13;
STUDY DESIGN: Cross sectional study.&#13;
PLACE AND DURATION: Department of Psychiatry, PNS SHIFA, Karachi from October 2009 to&#13;
April 2010.&#13;
MATERIALS AND METHODS: Fifty six patients with Schizophrenia reporting for management in&#13;
PNS SHIFA were enrolled as subjects. Demographic variables and other confounding variables&#13;
were recorded in a structured Performa, Blood Pressure (BP) and Waist Circumference (WC)&#13;
were measured while Fasting Plasma Glucose (FPG), Triglycerides (TAG) and HDL-Cholesterol&#13;
(HDL-C) levels were estimated by routine methods in laboratory with quality assurance.&#13;
RESULTS: More than half i.e.30 (53.6%) of subjects had Metabolic Syndrome (MS) as per ATP IIIA&#13;
criteria. Further 25% (n=14) of subjects were positive for two components of MS. Low HDL-C&#13;
levels were most prevalent component followed by hyperglycaemia, hypertriglyceridemia and&#13;
increased WC while hypertension was least common component.&#13;
CONCLUSION: Metabolic Syndrome and its various components are frequently co morbidity in&#13;
patients with Schizophrenia.

Objectives: The study was planned to find out frequency of anxiety and depression in patients suffering from spinal cord injury (SCI) and comparing it with normal subjects, to ascertain whether anxiety and depression were more common in patients suffering SCI.&#13;
Study Design: A case control study.&#13;
Place and Duration of Study: The study was conducted at Psychiatry, Neurosurgery and Rehabilitation Departments of PNS Shifa Hospital Karachi from Jan 2009 to June 2009.&#13;
Patients and Methods: Purposive, Non probability type sampling was used to select the study group. Group 1 (n=80) comprised of patients who suffered SCI and Group 2 (n=80) comprised of healthy hospital staff and healthy relatives of patients as controls. Hospital Anxiety and Depression Scale Urdu version was used for psychiatric evaluation. Data was analyzed by using Statistical Package for Social Sciences (SPSS) version 10.&#13;
Results: In group 1, 30% suffered from anxiety and 42.5% suffered from depression as measured on Hospital Anxiety and Depression Scale. In group 2, 6.3% had anxiety while depression was produced in 1.3%. Significant difference was found in the frequencies of anxiety &amp; depression between both groups.&#13;
Conclusion: Both depression and anxiety are more common in patients with SCI as compared with normal people which raises the need for screening the patients suffering SCI with a valid tool such as HADS for better treatment out come and reducing the disability.&#13;
